2020-2021 POLICIES AND FEES
Please read each section carefully!
All families will have access to our parent portal to process payments, register for classes, and be informed of current happenings at the studio. It is very important that you check this often and keep your information up to date. We understand that as a parent, you receive a million emails a day, but it is our most used form of communication and the best way to contact us. We are committed to staying “green” by reducing our output of paperwork. Please add to your address book to prevent important information being sent to your spam/junk folders.

Tuition is based off of the whole season from September to May and is divided into equal monthly payments for your convenience. August tuition is half of your regular monthly fees. You may sign up for auto-pay through the parent portal if you so choose. The first payment is due upon registration with the remaining payments due before the first of each month. Tuition does NOT reflect the amount of classes or weeks in month and is NON-REFUNDABLE in the case of missed classes, injuries, illness, etc. Holidays are not prorated.
Multi-student & Parent discounts: Families with more than one student enrolled at 5678 Dance will receive a 10% discount off of Monthly Tuition payments. Parents of enrolled students will receive a fifty percent 50% discount off of Tuition Payments for adult classes. The adult class discount is the maximum discount available no matter the number of enrolled students the parent has.
Late Fees: A $10 late fee will be added to bills unpaid by the 5th of each month. Please note: a student's account MUST be up to date in order to perform in any scheduled performance.
Registration Fees: $40 for the first dancer and $20 for each additional sibling due upon registration with your first month’s tuition.
1 class per week: $65 September-May; August - $32.50
2 classes per week: $105 September-May; August - $52.50
3 classes per week: $140 September-May; August - $70
4 classes per week: $170 September-May; August - $85
5 classes per week: $195 September-May; August - $97.50
6 classes per week: $215 September-May; August - $107.50
7 or more classes per week (unlimited): $230 September-May; August - $115
Thirty days notice is required for ALL students who wish to withdraw from classes. Billing for tuition will continue until the 30-day period has been completed. Please see our Withdrawal Policy section below.
Payment: We accept payments via credit/debit, cash, & check. You may also enroll in auto-pay on your parent portal via ACH or credit card. There is a $25 fee for all returned checks.
Authorization: I hereby authorize 5678 Dance LLC to charge/debit my account for payment of my bill for all activities, costumes, special events, etc. at 5678 Dance LLC. I further acknowledge that the additional terms of each transaction shall be subject to client’s work agreement, contract and terms and conditions on file, if no current work agreement on file, then the undersigned agrees that the compensation is determined by 5678 Dance LLC at their own discretion. I acknowledge that the services may not be processed without agreeing to the above.
Performances & Costumes
5678 Dance will present TWO fabulous productions this season: Our “Legendary” recital (postponed from May 2020) and our Annual Spring Recital. More details on each performance will be released closer to the show. All dancers will participate in recitals. If you know you will not be able to participate in either show, you must e-mail us at firstname.lastname@example.org by September 1st for “Legendary” and December 1st for our Spring production.
Costumes: Each class will have TWO costumes - one for each show. Costume prices generally range from $75-$100 and include a pair of tights. Costumes are due according to this schedule:
Fall performance costumes: Due September 1st (*If you are a returning student, this has already been paid.)
Spring Performance Costumes:
October 1st: Ballet, Pointe, & Lyrical
November 1st: Tap & Musical Theatre
December 1st: Jazz, Tumbling, Hip Hop
*These will be included in each month’s invoice. Late payment of costumes may result in a late arrival of your dancer's costume!
*Costume payments are NON-REFUNDABLE for any reason.
I give my consent for images (photographs, video) of my dependent to be taken and used to document the activities of 5678 Dance. I grant 5678 Dance permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent to be used, I will indicate this in writing and the signed letter will be given to the front desk.
Thirty days written notice is required for ALL students who wish to withdraw from lessons and billing for tuition will continue until the 30-day period has been completed. This will still be in effect if there is a government-mandated shutdown of our facility. If you are wishing to withdraw from classes, you must e-mail us at email@example.com before the first of the month to discontinue billing. Once tuition has been paid, it will not be refunded.
It is the responsibility of the parents to provide proper attire for class, but it is the responsibility of the student to dress properly for class. Students not properly attired may be asked to watch class. Hair should be pulled back off of the face and buns MUST be worn for ballet. Please write the student’s name in all shoes, bags, and attire. Please refer to the Class Descriptions and Attire section of our website to find the correct dress code for your class and where to purchase items.
Students should not be dropped off at the studio more than 5 minutes before scheduled class times and should be picked up no later than 5 minutes after class ends. 5*6*7*8 Dance shall not assume responsibility for students dropped off early or left late and shall not assume responsibility for students outside the building.
Discourteous or disruptive students may be required by the instructor to sit down and take a “time out” from class, or parents may be called to pick the student up early. Consistently discourteous or disruptive students and/or parents may be required to withdraw from classes at the discretion of the Owner.
No food or drinks other than water will be permitted inside the studios.
No devices such as smart watches, cell phones, tablets, etc, are allowed inside the dance studios. Cell phones in bags must be turned off. We do not permit the use of cell phones between classes unless there is an emergency. If they are seen, they will be given to the front desk until the dancer is finished with classes.
Please be on time to class and make every effort to attend each session. Attendance is very important in your child’s dance education as well as to their classmates. Make-up classes are available, but do not count toward perfect attendance.
We follow Gwinnett County Public Schools decisions on inclement weather closings/delays. You may make up these classes if you wish, but no refunds will be given.
We will be providing online classes simultaneously with our in-person schedule if you prefer that option so you may dance from the comfort of your home!
In the event of a government-mandated shutdown of our facility, we will convert to a fully online, virtual format to continue to provide our families with their regularly scheduled dance classes. If you wish to discontinue, our withdrawal policy remains in effect and we must have a 30-day notice on file.
We follow Gwinnett County public schools’ calendar as well as their decisions on inclement weather closings and delays.

IN CONSIDERATION OF BEING ACCEPTED AS A STUDENT IN THE DANCE COURSE, THE UNDERSIGNED ON ITS BEHALF AND ON BEHALF OF THE STUDENT RELEASES AND DISCHARGES 5678 DANCE AND ALL ASSOCIATED STUDIOS, ITS AND THEIR OWNERS, EMPLOYEES AND AGENTS, FROM ALL CLAIMS, DEMANDS, RIGHTS OR CAUSES OF ACTION, PRESENT OR FUTURE, WHETHER KNOWN, ANTICIPATED OR UNANTICIPATED, AND RESULTING FROM OR ARISING OUT OF, OR INCIDENT TO, THE UNDERSIGNED’S OR THE STUDENT’S USE (OR INTENDED USE) OF A DANCE COURSE STUDIO OR LOCATION, OR FACILITIES AND EQUIPMENT IN SUCH A PLACE, OR AS A RESULT OF, OR INCIDENT TO, ENGAGING IN DANCE COURSE EXERCISES OR OTHERWISE FOLLOWING DANCE COURSE INSTRUCTIONS ANYWHERE.
ASSUMPTION OF RISK
BODILY HARM INHERENT IN ENGAGING IN THE DANCE COURSE, SAID PHYSICAL BODILY HARM INCLUDING, BUT NOT LIMITED TO, BRUISING, BROKEN BONES, AND MUSCLE, JOINT, NERVE, AND/OR SPINAL INJURY. MOREOVER, I UNDERSTAND THAT DUE TO THE NATURE OF CONDUCTING A GROUP DANCE CLASS IN AN ENCLOSED ENVIRONMENT WHEREIN STUDENTS ARE PHYSICALLY EXERTING THEMSELVES THAT THERE IS AN INCREASED RISK OF CONTRACTING A VIRUS, DISEASE, OR INFECTION, INCLUDING, BUT NOT LIMITED TO COVID-19/CORONAVIRUS EVEN IF 5678 DANCE EXERCISES GUIDANCE FROM THE STATE OF GEORGIA, THE FEDERAL GOVERNMENT, AND/OR HEALTH OFFICIALS.
UNDERSTANDING THESE AFOREMENTIONED RISKS, I HEREBY ASSUME ON MY OWN AND ON BEHALF OF THE STUDENT ALL RESPONSIBILITY FOR AND ALL RISK OF DAMAGE, INJURY OR SICKNESS THAT MAY OCCUR TO THE STUDENT IN 5678 DANCE’S COURSE OF INSTRUCTION, WHILE ATTENDING CLASSES, PARTICIPATING IN EXERCISES, USING DANCE COURSE EQUIPMENT OR FACILITIES OR FOLLOWING DANCE COURSE INSTRUCTIONS IN OR OUT OF THE DANCE COURSE STUDIO OR LOCATION.
